Blue Earth County Public Health offers assistance to elderly and/or disabled persons looking for community resources.

MN Choices Assessment

Blue Earth County Public Health offers MN Choices Assessment services designed to help people over 65 years of age and older make decisions about care needs. Social workers and public health nurses work together in teams to assess elderly or disabled individuals for services and supports that reflect needs as well as preferences.

The MN Choices Assessment is completed in the individual's home at no cost. Persons who are financially eligible are offered the following state programs: Waiver Services, Alternative Care, or Essential Community Supports designed to help with the cost of services in the community.

Public Comment for Amendment to the Elderly Waiver Plan

The Minnesota Department of Human Services (DHS) requests public comments on a proposed amendment to Minnesota’s Elderly Waiver Plan (PDF) before submitting the amendment to the Centers for Medicare & Medicaid services (CMS) for approval. This amendment updates the CDCS service definition to include shared services language to personal assistance. The amendment also provides other waiver services clarifications and other technical changes.
